So the design has never changed since it was made? The engineers have never needed to figure out an upgrade or slightly different way of doing it?

I submit that this is happening in a capitalistic environment, and there is a high probability that the safety mechanisms have not been maintained. Therefore, it is possible that the dead man’s switch has malfunctioned, especially if the train is hauling hazardous materials through Ohio.
